The links below contain information about Court Forms needed to make applications and other applications relating to family cases. At the bottom of the page are links to the Family Procedure Rules which set out the practice and procedure to be followed in all types of family proceedings.

Apply for a court order to make arrangements for a child or resolve a dispute about their upbringing: Form C100

All parties in cases involving children (except where the local authority has applied for an order) or finances on divorce must complete this form no less than 7 days before the first hearing in the case: Form FM5
