Apply for a court order to make arrangements for a child or resolve a dispute about their upbringing: Form C100
All parties in cases involving children (except where the local authority has applied for an order) or finances on divorce must complete this form no less than 7 days before the first hearing in the case: Form FM5

Make an application in existing court proceedings relating to children: Form C2
Ask for permission to start proceedings, or to request an order or directions in existing proceedings, or to ask to join or leave proceedings.
Get an injunction if you've been the victim of domestic abuse.
Give notice of your intention to proceed with an application for a financial order relating to a divorce: Form A
